The Health Assessment Physician Report is designed to facilitate communication between doctor and patient.

Mayo Clinic Health Assessment participants now have access to a new Physician Report to facilitate more meaningful, productive communication between patients and health care providers. The report takes the most meaningful information from the Health Assessment and lists this info in a physician-friendly format that a person can take with them to the doctor. With this information at the health care provider's fingertips, he or she will have a snapshot of the patient's overall health.

The Health Assessment Physician Report will be three to four pages long, depending on the participant’s survey responses. Contents of the Report include:

  • Perception of health
  • Reason for visit
  • Medical conditions
  • Medications
  • Allergies
  • Social history/habits (alcohol, nutrition, exercise, emotional health, etc.)
  • Biometrics (height, weight, blood pressure, glucose, cholesterol, etc.)
  • Identified risk areas
  • Immunizations and screenings

The Physician Report will appear on the Action Plan upon Health Assessment completion. The user can click on the Physician Report icon to open the PDF, which can be saved or printed.
